DELAYED REPORT OF BIRTH (If the birth was reported beyond the twelve (12)-month reporting period)
Both the Report of Birth and passport application of newborn infants/newly registered applicants can be completed on the same day. Please ensure that all the required documents listed below are fully complied with:
- Personal appearance of the child/applicant of the Report of Birth (ROB).
- Personal appearance of either parent.
- ENCODED Report of Birth (ROB) Form without signature.

- All entries must be CAPITALIZED.
- Item No. 4: Indicate the state, province, country (i.e. AL AIN, ABU DHABI, UNITED ARAB EMIRATES)
- Item No. 8: Indicate the birth order of the child to be reported (i.e. FIRST, SECOND, THIRD, etc….)
- Items No. 9 & 11: Indicate the full maiden (unmarried) information/details of the mother.
- Item No. 14: Follow place of birth and country of birth as shown in passport details.
- Item No. 16: Indicate N/A if both parents were born in the Philippines
- Original and four (4) photocopies of the UAE issued Birth Certificate (English Version) bearing the UAE Ministry of Foreign Affairs authentication stamp;

- Four (4) photocopies of the parents’ valid passports with signature;
- Original and four (4) photocopies of the Marriage Certificate/Report of Marriage issued by the Philippine Statistics Authority (PSA).
- Fill out the Affidavit of Delayed Registration Form.
- Negative Certification of Birth Record from the Philippine Statistics Authority (4 photocopies); and,
- Other requirement/s requested by the processor to determine proof of citizenship, identity, or eligibility for registration of birth under Philippine Laws.

- 140 AED for the notarization
- 140 AED for attestation
- 140 AED for the Affidavit of Delayed Registration
* Clients who opt to pay the express fee of 40 AED will have the ROB released within the day, after which the Philippine passport may be applied on the same day.
