LOST ePASSPORT
Prepare the following requirements:
- Accomplished ePassport application form (Adult) or ePassport application form (Minor)
- Report of Loss from the UAE Immigration Office, duly stamped by the Abu Dhabi Police General Headquarters, with English translation. Make sure that the passport number and the name of the passport owner are stated in the report. (Original and photocopy)
- One (1) photocopy of the lost passport, if available
- Philippine Statistics Authority (PSA) issued Birth Certificate (Original and photocopy)
- For married woman who opted to use her spouse’s last name, Philippine Statistics Authority (PSA) issued Marriage Certificate (Original and photocopy)
- Affidavit of Loss filed at the Embassy. Please refer to this link : https://abudhabipe.dfa.gov.ph/sample-sites/authentication-services
- Passport fee of AED 600.00 for lost ePassprt
- Passport fee of AED 360.00 for lost MRRPs (green passports) and MRPs (maroon passports)
Note: There will be a 15-day clearing period prior to the processing of application for replacement of a lost valid passport.
Additional Requirements:
In case passport was already declared lost by the applicant but eventually was found and returned to him/her within the 15-day clearing period:
- Submit Notarized Affidavit of Retrieval (with detailed explanation on when, where and how passport was found)
- Original and photocopy of data page and last page of passport

MUTILATED OR DAMAGED ePASSPORT
Prepare the following requirements:
- Accomplished ePassport application form (Adult) or ePassport application form (Minor)

- Passport fee of AED 240.00
